I shoot similarly to Russ... 3 incadescent lamps that move to angle the light correctly... the rest is just a good macro camera and a push of a button.
I use a Nikon Coolpix 4500... very good camera... I love it
Nothing really earth-shattering, just a lot of hard work. I use an old Ricoh RDC-4300 that can be had for about $60 to $70, GE Reveal light bulbs that cost $4.99 a four pack, a copy stand ($41), and three desk lamps ($6.99 each):
The key is angle, and the lighting and using the appropriate number of bulbs and wattages depending on the type of coin being shot. I just move the lamps around until the coin looks correct in the viewfinder.

Also, try not to shoot through holders... slabs are one thing, but if the coin is in a holder like this, take it out... raw pictures are EASY
