  Looks like an extreme case of machine doubling to me. It's neat though.

  Change that we can believe in is that change which is 90% silver.

  Looks like yours might be the K05-50 Triple mint mark.

  I thought machine doubling also, but I've never seen it tripled and it's only on the mintmark.

  That looks like machine doubling to me. I've found others before where only the mintmark was machine doubled.
  
  Here are a couple other reference sites. Neither has a tripled mintmark for 54S.

  Looks like machine doubling damage to me, and yes it can cause tripled or even quadrupled images.
