Did you ever buy a coin that was graded but had to remove it due to PVC appearing later on?

Heard of it, never seen PVC grow over time with my own eyes. Cracking a coin to remove PVC would be the safest way to preserve the item in question.

Can PVC create a tiny greenish crystal on a coin? Or is that verdigris or both can do that?

PVC goo doesn't just "appear" out of nowhere. It essentially has to be squeezed onto the coin by prolonged direct physical contact with phthalate-laced PVC. Now, the goo might originally start out as transparent, which a TPG might not notice when grading the coin, and later develop the green colour as the acid in the goo slowly reacts with copper in the coin's surface.
I'm not sure if "not noticing PVC goo which later turned green" is covered by the TPG guarantee. A coin covered in PVC goo (whether it's green or clear) is supposed to be either returned as nongradable or sent down for conservation, depending on what the customer paid for.
But if a coin has PVC goo removed with acetone, that goo cannot reappear without the coin being put back into a PVC coin album. A slabbed coin should be safe from this.

Yes. I recently cracked out an 1819 Bust Half in PCGS AU58 CAC due to active PVC on the coin. It has since been conserved. Naturally the coin didn’t come back 58, so now I have to send it in again with proof it was in the holder. Yes, I could’ve just subbed it for restoration and waited a couple months and been charged by PCGS for doing what I could do for free. Gamble on my part but whatever.

Agreed. I've seen many coins in PCGS or NGC holders rejected by CAC that JA puts a red sticker on that says PVC both live and many on YouTube by legitimate dealers who submitted. Most of the time he spots it at the white haze state, not the green goo that hopefully anyone will spot and not slab -- but I am sure that has happened like everything else bad that can happen.
I am not sure what service slabbed it, but a couple will basically do it for free if they agree with your diagnosis. The grade will be guaranteed so if they lower it a partial reimbursement is due. Contact their customer service for details/procedure.
I'm not sure that is correct. Part of the problem is grading services sometimes can not see the white haze PVC that CAC sees.
