eBay Authenticity Guarantee - What does PSA do when they find a fake holder?

I'm seeing a high-value PSA-graded card on eBay that looks a little off to me.
I did a Cert# Lookup, and the grade is correct but there's no picture of the card to compare to the one in the eBay listing.
The Barcode on the front and the back (as well as the QR Code on the back) all bring me to the PSA graded card.
I don't see evidence of the card case being cracked open (like frosting around the edges).
What would PSA do if they flag this during Authentication? Do they invalidate that serial number? What if the original owner has his real version of this card, and the serial number is taken down? That would be bad for the legit owner.

How is the auction worded? A lot of times the scammers know how to avoid the authenticity guarantee by re classifying the auction or category.

I bought something the other day that I thought should have been authenticated but wasn't. Do auctioned items go through authentication? Or only buy it now items?
"Items with a Buy It Now price of at least $250 with either auction format or Best Offer are also eligible." The key word is eligible. You could buy a Mickey Mantle rookie, but if it's not eligible then you don't get the authenticity guarantee.

Am I correct in assuming if I start an auction at 99 cents and it sells for over $250, it goes through the authenticity guarantee?
The eBay listing has the Authenticity Guarantee (as well as PSA Vault Eligible). My concern is the slab looks legit, but the card is questionable So this made me wonder what does PSA do in this case if they determine the card in the slab is fake. Do they deactivate that Cert#?
It will be marked with blue dot if authenticity guarantee is eligible. It's not uncommon for the same card to have a guarantee in one listing and not in another listing.

I think the terms have changed since the program launched.
I thought any card that sold over 200 dollars was automatically processed through PSA.
What items are eligible for Authenticity Guarantee?
In the US, Authenticity Guarantee is available for individual sports trading cards, collectible card games, autograph, and relic cards, and non‑sports trading cards $250 and above for ungraded cards, and $250 and above for autograph, relic, and other graded cards. Only cards graded by the following graders will be accepted: Professional Sports Authenticator (PSA); Sportscard Guaranty Corporation (SGC); Certified Collectibles Group's affiliates, CGC Cards; and Beckett Grading Services (BGS). Sets, lots, kits, decks, boxes, packs, art cards, and raw redemption cards are not eligible at this time. Your single card must be listed in the correct corresponding category to qualify for Authenticity Guarantee:

Impact of Fraud Detection on Buyers and Sellers. You acknowledge and agree that if the Authentication Partner detects fraud or suspects that an item is counterfeit, the item will be removed from marketplace circulation and, if the card was graded by PSA, PSA will deactivate its certificate. If the slab is counterfeit or tampered with but the underlying card is authentic and has not been compromised, PSA may re-holder the card and create a new certification before sending the card to the buyer. In addition, eBay will work with the proper authorities as needed and reserves the right to confiscate or destroy any items determined to be counterfeit.

I believe that any card listed for minimum $250 will go for authenticity even if the seller accepts a BO

Based on the Policy, Thanks athleticsfan, a seller can avoid authentication by listing out side of the 3 categories listed. The average LIV buyer wont be able to discern a difference... I expect one NJ seller to exploit that loophole.
