Has anyone ever seen this error on a PCGS holder before? The date base has it valued as a Trade Dollars

Definitely would be worth more if it was a Trade $.
That's cool... can't be too many of those mistakes floating around.

mistakes on labels are not unheard of
I imagine it wasn't long after they first fired up their slabifier that the first mistake was slabbed. I guess now the quest is on to find the earliest surviving example.
