2006 P North Dakota Washington Quarter Error

My best friend just came across this quarter error with major die strike errors on obverse and reverse. I have researched known errors and nothing seems to be similar. What looks like a "salmon head" to the left of Washington's face is pretty strange. Any help from the community would be appreciated. We are dumfounded by this.

No errors, just damaged from heat. Often called a campfire coin.
