The original didn't have In God We Trust while the 2009 UHR did have this motto so the designs are actually different.
They could have put the motto on the edge to retain the original design as well.

There was one of these recently found in Europe in a flea market for cheap, and was graded pcgs ms62. Coinworld did an article on it earlier this year.

I dont have much knowledge of that being a medal.
It was the 1st pattern coin struck for the US. Pewter, Brass or Bronze and Silver iirc.

The original Sacajawea with a W mint mark like the 22k pattern coins.
St. Gaudens or Coronet double eagle designs on bullion with appropriate relief.
Something big....Ike with eagle reverse.
I believe it's considered a European medal now, not created in the US.
