CAC Census question

For certain coins, I follow changes in the CAC census. One date had the highest grade stickered as MS-60 and there were five at AU 58. A year later the census reflects 1 still at MS 60 but only 4 at AU 58. What could cause the decrease from 5 to 4?

1)Someone returning the tag so the cert # is no longer valid
2) cac choosing to remove the cert# and remove the sticker as they feel they made a mistake

I can think of at least three scenarios.
If the coin were stickered as an AU58 and then regraded and evaluated again it might receive a second sticker. Later, the naked cert could be shown to CAC to have the first instance of that coin's evaluation removed from the census. Alternatively, a coin crossed from PCGS to NGC (or vice versa) might generate a CAC sticker upon both evaluations of the AU58 grade, but again if the naked cert were shown to CAC they might remove one instance. Lastly, if a coin has a CAC sticker and is then cracked out of its slab and the naked cert shown to CAC they again may remove it from their census.
