1925-S Peace dollar. Mintage of 1.6 million. The highest grade is MS65, and there are only 42 of those with many crackouts and re-submissions I'd imagine.
I'm sure that's not what you're after, but the 25-S is a poorly made issue relative to the rest of the series. It is a condition rarity, and along with the 28-S, the assembly of a gem set gets tough.
The top grade for an 1861-S $20 Paquet Reverse has to be below mint state as no mint state examples are known to have survived despite several thousand leaving the San Francisco mint before their exit was halted.

I agree! So crazy to see price jump from 62-65 each and every grade!
