it looks like either extreme failure resulted in cracks or else it was cancelled and then re-used.
Is that into or out of the surface? In the pic it looks like it's protruding as if they canceled the die but still made the bar using it. I know it's just an ounce bar but I think I would be suspicious of its composition.

Cancelled die strike. Some bar manufacturers struck off a few to prove that the limited-edition die was cancelled and/or to sell as novelties.

As the Captain stated it is a cancelled die strike. Years ago they were advertised for sale, but I haven't seen one in a long time.
