  Looks like a horrible case of hemorrhoids to me. Not that I know what they look like, just a guess...

  Actually it appears to have originated in the hind quarters as a lamination that fell off and got struck
  into the coin in a different area. Note the heart shaped depression at the buffs hip bone.
