If you do a Google search you will find a thread from April of 2015 discussing the new holder with the hologram. Sometime after that would be a reasonable guess.
ETA: FWIW, I subbed several in April 2016, popped July 2016, with serials starting 256202, so this one is sometime after that. My May 2016 subs were returned starting with 258 serials, but I didn't note the poppage date.

Are the certification numbers assigned sequentially?
They have been since before then. It's likely someone submitted that one at a higher service level sometime in April after my sub was logged. Pretty sure the cert #s are assigned at loggage time.
As for the sleeve, I’ve had two of the same standard sized cards graded, both 10’s. One came back in a regular holder, the one that was /25 came back in the Mylar sleeve. No rhyme or reason why. Slabbers choice I guess. I don’t mind the sleeve, keeps the card from moving around in the holder.

April 17th they were rolled out.
Have any changes to the holder been made since that date?
Did PSA recently beginning putting cards in plastic inside the holder?
Is there anyone at PSA I can email that can tell me when the card was submitted to PSA?

On June 14th of this year an article was put out highlighting the newest change.
It is hard to say on the plastic sleeve. I have cards from the same set where some have it and others do not. I have been told it is up to the sealer.
I am not certain if PSA keeps that information and if they do if they share it with the public. Perhaps you can call in and request it.
Holder, I don't believe so, but the flips, yes.
PSA sometimes puts cards in the plastic sleeve, typically if they are slightly oversized, but no real rhyme or reason to it.
