Some PSA form filling help

So I got three Clemente cards on eBay yesterday, and I want to get them all graded. Looking at the form, I have one major question regarding the number of orders vs. the number of cards:

• So if I just want to get 6 cards graded at the same service level, would that be 1 order and 6 cards?

And I'm assuming the price is 6 * $18 not 1 * $18, right?
