Two errors?

ok i have this 1978 Indonesia 100 rupiah which has a partly filled A but what is going on under the A? looks like a bit of metal peeled off or it's a die crack one of the two

    I think the "two errors" are two symptoms of the same event: a piece of metal has peeled off from beneath the A, and bent itself up and over into the hole.

    thanks for the info was wondering what had happened and i gather i could call it a Lamination filled letter error
