Any help on this Arnold Palmer piece?

I picked the piece up about 10 years ago treasure hunting in a second hand store for next to nothing. I have no idea how it was distributed (I would guess a sporting goods issue in the 60's). Any help on this would be fantastic. I hope that the 2 photos upload okay. Thanks and Happy Collecting.

Forgot to add that the disc rotates and diagnoses your golfing problems.

I collect a lot of golf cards and miscellaneous and have never seen one of these. It's not mentioned in either of the golf guides I have either. Looks like something that might have been distributed with a clubs purchase or from a pro shop. Great item!

They're not that uncommon. I found about 5 of them on ebay, and they all sold for about $10.
