So with the headings, does PSA decide what to put on there or does submitter? I see older tickets with hardly any info so does PSA know what the ticket is for or do they rely on submitter?

On the form you document what significant event happened in the game, but PSA reserves the right to use that or not on the flip. They do their own research on each ticket and ultimately decide. But what I wrote on the form and what ended up on the flip was fairly spot on.
