Issues with on-line submission form?

Does anyone else have an issue with trying to bring up the PSA On-Line submission form?

I get the flowing:

The requested page was not found.

We're sorry, but we can't seem to find the page you're looking for. There is no need to report this error as an error incident report has already been sent to the system administrator.

I first got the "this page cannot be displayed", but my computer was running slow. Rebooted, now it's fast and the page came right up.

If you go thru "Services", it works fine. If you go thru the monthly special section, it does not work for me...
