Why do some people have no avatar/icon picture, but instead "Author Icon" that links to the user's profile?

The ones I've seen are for members who had pictures before.

My guess is that their avatar got lost in translation when the switchover occurred.

It happened to me. I went to switch an icon, to do that you need to remove your old icon and add a new. Then you press save. They probably have a Sig line that has characters that as of about a week ago stopped being allowed so when they press save they get an Error 113, they then go to try to put their old icon back and do the same thing but will find that they keep getting the error and don't know what it means. Again, I was stuck in the same situation and through a lot of trial/error figured out the problem.

They should a) delete their sig info, b) save, c) add a new avatar, d) save, e) add a sig line that complies with the new functionality and voila...they have both. But even if the new sig line doesn't comply, they will still have an avatar because they would have saved it before their sig line errored them out.

So net-net, they need to delete their sig line and add an avatar, that will get them their avatar...and then after they can play around with their sig line.
