I am "flipping" out

I can't figure this one out. The card is an authentic 1996 Leaf Signature as it has the foil stamp as only the autographed cards have. The certification number brings up something else altogether, a hockey card: 1985-87 Hall of Fame Cards Bill Chadwick #65. Could it just be a mechanical error?

It has happened a couple times to me before..

It is most likely a clerical error. I have seen a card registered incorrectly like your example or registered correctly but the grade is wrong. PSA customer service will take your information and get back to you with an answer.
