Amazing! What was the circumstances that you saw this die? Who owned the die at the time?
The 2011 Martha Washington test pieces are now assigned numbers Judd-2225 to Judd-2233.
The only known pattern of this design in private hands is a waffled one owned by forum member Mike @Byers.

Is that your die and is the face that of the MW pattern that says "First Lady" under her bust?
There aren't too many waffled canceled patterns. Here's one I just ran across for what looks to be a clad quarter:
