Question about + grades

What do you look for in a coin that says to you that it is worthy of getting a + grade?
Is it the strike, luster, toning, etc...
I noticed that on occasion there is a coin such as one minted in the last year or 2 that receives a + designation. What would cause PCGS to give this grade out?
