Sports Q&A Card - Oddball what is it?

I'm trying to identify the who made it, when (narrowed to somewhere between 1985 & 2002), and how was it distributed of this card. I haven't seen any others, but got this in a golf lot recently. It's slightly taller than a PSA slab and about a 1/2 inch wider. I'm guessing it's from closer to the 1985 date or the Bossy answer would have included the 85-86 season. Sam Snead passed away in 2002, so it has to be before that.
