Would this fall under a reholder sub?

Just received this card back from one of the group subs. Noticed yesterday that the card number was incorrectly labeled. Card was labeled #19 when it should have been labeled #112. If I wanted to send this in to fix, would I request a reholder and what form do you fill out for that? Here are scans of the front and back:

Don't know the answer to your question but I wonder if PSA got it wrong of the group sub leader entered the wrong card number on the form?

Yeah I am not sure. As you can see from the back of the card, it is a little confusing with the #19 after Sakic. I may have mismarked it when I shipped it to the submitter, or maybe the submitter made the mistake. Could have been the grader. Not sure, but would like to get this labeled right.

Mechanical error. PSA will correct it for you.

Edit: Response back from PSA for one of my past mechanical error claims:

Please fill out a PSA submission form, mark "Other" as the service level, write in "Mechanical Error" and include a short note on the form describing the error. There is no charge or return shipping fees for the correction.

That mistake is known as a 'mechanical error'. You can simply send it back to PSA with that notation
on the grading invoice. What sucks is you have to pay the shipping to PSA.

You can get a voucher to cover shipping to PSA if you call CS.
