Did PSA make a mistake holdering this card?

Just purchased my first graded vintage card. When looking at the back, noticed it looks like there is a plastic film between the back of the card and the holder. Not sure how well it shows in the scan, but you can definitely see it when held up to the light. Front of the card looks fine, this is only on the back. Could PSA have possibly put it in the holder with part of the penny sleeve still on the back? Checked my other PSA graded cards and none of them are like this. Any opinions would be appreciated.

In the 1960s Sprite issued bottle caps with AFL players on them. I recently called PSA to see if they would grade and slab these. They said they didn't have a case to hold them. When I hung up, I measured a standard bottle cap: 1/4" thick. PSA has slabs for 1/4" thick patch cards. So I called back to double check whether that was an option. The woman said the cap would move around. I said I've seen you put tiny cards in much bigger slabs and they don't slide around because you put in a Mylar liner to prevent movement. Couldn't you do that with the bottle cap? I never received a satisfactory explanation why they couldn't slab the caps, but she was very nice and I didn't want to appear rude, so I gave up. Frustrating though.
