I pulled Rickey Henderson and George Brett in the same case in 1991. It was two boxes and got one in each box.
I am not sure I ever pulled another one in probably 100 boxes in that 20+ year period.
** Working on the following sets-2013 Spectra Football Hall of Fame 50th Anniversary Autograph set, 2015 Spectra Football Illustrious Legends Autograph set, 2014-15 Hall of Fame Heroes autograph set. **

Along the same lines I think 1991 topps set cards were even less than one per ten cases and I pulled two 1975 topps commons in back to back boxes. I'll never see another topps set card in a 1991 box again lol.
Odds are long. I opened 1 sealed case and half of another and didn't receive one back in 1991. The next year, 1992, my buddy who owned a chain of convenient stores came by my house one day and had a single box laying in his truck. I asked him what he was going to do with the box. He told me nothing in particular. Maybe stick it in his store. About that time, he reaches over and grabs it, hands it to me, and says 'if you get anything good, you can give me $30 for the box'. After he left, I started busting packs and hit the mother of all Elite cards at the time, the Frank Thomas. It booked for like $400 which was big money back then. Great memories..

I recall hearing about one pulled from a shop years later when it was junk wax and after I read that those cards had metal in them and people had used detectors on them. Not sure of the truth in that though.

+1
My holy grail was a 1992 Elite Frank Thomas with the serial number 00035/10000; it wasn't pulled until 2011 and I was lucky enough to purchase it.
