Need help identifying a Campanella card

Hey guys,
I have had this in my collection for probably 20 years and have no idea what it is. I haven't tried to ID it for a while. Any idea what set this is from? Possibly a collector issue? It's got his complete lifetime stats, so this has to be from after his playing days. Card measures 3 1/8" X 5".

Thats where I got a lot of my Dodgers LAPD cards...they used to put out sets each year.
Actually now that I think about it, I think it was in a fan club package. My grandmother did get it for me at an LAPD-Dodgers co event.
I did not know about his condition, so in the early 1980's I wrote to him requesting his autograph and that is what I got. So I can say with confidence that the card is from the 1980's given out for autograph requests.
I wrote to many players in the early 1980's and got a great deal of responses including Hank Aaron, Willie Mays and Stan Musial. I am quite certain that they are all good but I will get them certified one day to make sure they are legit and not ghost signers. I still remember the first response I got was from George Foster and was disappointed the Johnny Bench was an auto pen.......at least I believe his is an auto pen.

I still remember when dad brought home this gentleman's collection he was going to help sell for the widow. There was a Babe Ruth auto on a ball, plus many other Hall of Famers from that era. I can say that I actually held a Babe Ruth signed ball. The leftover stuff that I got was pretty cool. No Babe Ruths or Ty Cobbs in that bunch, but there was close to 1,000 autographs of players going back to the 1920's, including several Hall of Famers and a Roger Maris signed 3 x 5. I still have them all.
