Need help identifying card....

I can't seem to find anything about this card, Bib Falk Chicago White Sox. Doesn't seem to match anything in the Pop reports.

Any ideas?

Have been doing some research on the odd ball cards for a while, the W572 Falk can range anywhere from $15 - $100, depending on condition of course. Great find for $5 !
