What does PCGS mean "Genuine"?

Ebay #380967594673. That is all it says on the slab. Why didn't they grad it? If it were cleaned, wouldn't they say so?

Code 92, which I think is cleaned.

PCGS does not list the problems for some coins. That is also described on the link given above.

I forgot, PCGS has the option on their submission form to NOT list the details grade. Perhaps that is what the coin in the OP was submitted under. When I have used the service, I ask for the details grade. I do not really know why anyone would ask for it otherwise...

"Genuine" means that it's real, i.e. not a fake, but also is not "Market Acceptable" to receive a grade.
