Mechanical Error Re-holder

What happens with cards in your inventory when they undergo a re-holder for mechanical error? Are they removed from your inventory and you need to re-add them or is their description just updated and they remain in inventory with your previous notes and images attached? Just curious as I'm awaiting a mech re-holder to POP.
