Nice Koufax item reacquired

I bought one of these in the late '80's. In 2001, I sold it on eBay. Lately, I've been lookling to get a nice one at a reasonable price and finally found one. Still sealed in the original cellophane. It apparently was issued in 1964. I have seen several Yankees on eBay as well as Koufax (and nobody else). It is a cloth patch with a cardboard backing. If anyone has additional info on this issue, I'd love to hear about it.

Looks like the 'set ' consisted of 8 Yankees and Koufax. All the ones for sale on eBay cite 1964 as the year of issue. Can anyone provide any other information? Where/how they were sold, etc?

Can't find much data on these. The 2011 Standard Catalog has the date as 'approximate year of issue' and notes their checklist is incomplete. Total of 10 players listed: Berra, Boyer, Howard, Koufax, Kubek, Mantle, Maris, Pepitone, Richardson, Tresh.
