Thanks man. This one is pretty obvious, but I have a few others that need corrected. Do you think I should leave a post it note on each one explaining the error?

You need to send it/them in with a submission form filled out as best you can explaining the problem(s).
Write "mechanical error" boldly on the form. PSA should fix them at no charge. Make sure you enter a value for the item(s) as well.
Contacting customer service before hand wouldn't hurt.
