If you see + feedback left for a card that you know was 100% fake would you let the buyer know? I believe ebay would not refund them even if I did because they + feedback but they got burned for over $400.

Yes...I hate to see people buy fake cards and get conned out of good money. They will most likely get disgusted with the hobby, quit buying cards, and that will be one less person to buy my goods.

You can definitely still get a refund after leaving positive feedback as long as its within the 45-day window.
