Couple of grading questions?

I attempted to ask some of these questions to customer service via email but either I was drunk and was not clear enough or they were loaded and could not understand me?
From reading many of your posts lately, I know there are a lot of knowledgable folks out there and I can probably get some good answers.
1) If you send a card in an old holder in, wanting to get a half point upgrade, and the card receives the same grade that it had, does PSA put it in a new holder or just sends it back in the old holder?
If they put the card in a new holder with the same grade, that's fine. But if they don't, how would you know if cards that you buy in old holders have been sent in for review in the past?
Point being, I don't want to waste money and send in a review that has been sent in 2 or 3 times before.
2) Am I correct in referring to the above example as a "review"?
Because I have been buying cards already graded lately, I have not renewed my membership.
Do you have to be a member to send cards in as "reviews"?

Yes, you have to be a member to submit reviews. I've been waiting over a year to renew so I can submit my collection for reviews, but won't until they run another review special. Which may be never.
That's one (of a few) reason I've never quite understood why reviews cost the same and full grading. They should at least give you something back (vouchers would do) if they don't re-holder the card!
