Question on PSA graded unopened packs

I am not totally clear what exactly is being graded when PSA slaps a grade on an unopened pack with a star on top. Are they looking at the over quality of the cellophane/wrapping material? Or are they looking at the star card on top or bottom? Or are they looking at both as in the overall package?
