Need some help please...

I have been "lagging" in a couple of recent trades. I also know that in my 2 year online card tradings I have been ripped off a couple of times, and didn't relize it because I keep no records. I was wondering if some "organized people" would be able to help me. I also need some tips on some other subjects. Any help is greatly appreciated. Here are the things I need to know:
1. I am looking into getting a note-pad to log all of my trades I have done. What kinda of "columns/categories" do I need to have on the note-pad (i.e. person I traded with, card I am trading, etc.).
2. I am in desperate need of "organizing" my cards. Are there any "tricks of the trade" I need to know? Such a binders, boxes, etc.. What is the best way to organize them buy? Team, player, set?
3. Need a little help on packaging inside of a bubble mailer, I have always just thrown the card in the mailer inside a penny sleeve and then into a top-loader. What type of packaging would you suggest inside of a bubble mailer? Possibly inside a regular envelope in the mailer??
4. Sometimes I run out of newly bought bubble mailers. Do you think it would be acceptable for you to receive a package from me that I got in a trade from another person (that is all patched up). It has happened only a couple of times, but at the time I figured it was ok as long as they were sent on time!
5. I have always heard to insure when the BV is $50 and up. Should I be insuring the package at a lower BV such as $30 or $40, or even increasing it to possibly $60??

I have an outline on my comp that i fill out each trade. It lists Date, name, address, ID and/or email address, cards traded, cards traded for, and my email address. I ame 2 copies.. 1 for me and 1 for inside the package. I have received from people who have used old bubble mailers.. and i dont care.. as longs as the cards get to me in good shape... i am happy.I rarely insure... for the $$$ it costs to insure.. it isnt worth it to me... but thatsbecause i live incanada..lol
