How do you add more than 20 slots to a PSA submission form?

I want to take advantage of the May special for bulk orders, but the submission form only has 20 slots for cards. Is there any way to add more slots to the form?

You can create your own in Excel and attach it. Someone here also has a link to print additional sheets all the way up to as many as you need.

You can use multiple sheets. I was told you could submit on a spreadsheet as well.
