Hawaii Extra Islands Statehood Quarter

Can someone tell me what caused the extra islands design on the 2008 Hawaii Quarter, what is the mintmark and are the error coins available? There was an article in NN recently.Thanks, Mark. PS: the article referred to these 2008 quarters as interesting just like the Wisconsin Extra Leaf Quarters minted in 2004.

    Holy Crap™, there's going to be a lot of "[sic]" required by anyone who quotes that article.

    Without having seen the coins in question, my bet would be that the extra islands are die chips, not "dye [sic] gauges [sic]".
