Environmental Damage??????

I received the results from one of two submissions today. One of the coins was body bagged for "environmental damage". The coin, a 1964-D quarter was taken direct from a P&D mint set in original packaging. I removed from unbroken cello and placed directly into a safe-flip for submission to PCGS (I even wore new cotton gloves).

Could the original packaging cause this? Coins shipped today so I will post a pic once received........

Images would be really helpful. What you're asking shouldn't happen, but...

Next time submit it in the cellophane pkg?
