PSA/DNA Submission Form Question

I had a question I was hoping I could get answered. I am submitting my first order to PSA this week and I had a question. I would like to know where to put the Card details
" Brand Name, Year, Card #"? I am assuming that it is goes under the Primary Autograph listing, but do not know. Do they identify the card for you? Any help would be appreciated. Thanks.

If there is not enough room you can always reproduce the list in excel and attach your own sheet.
The first thing you should understand is the difference between red flips and blue flips. On red flips, both the autograph and card are authenticated (and graded if you want). On blue flips, only the autograph is authenticated (and graded if you want). The card could be counterfeit or unlicensed or trimmed and it wouldn't matter.
Use a PSA/DNA submission form for blue flips. Use a standard PSA card submission form for red flips.

Thanks for the question. Yes the cards were personally signed. With this months special on Blue flip submissions I was going to go ahead and get these authenticated, since the price increase, you never know if there will be a better or same deal come along. I thought I would go ahead and get them authenticated. All I am looking for is Authentication and to get the cards protected, in a slab. I welcome all suggestions. My biggest question is how do I make sure the card details are on the slab once it is authenticated. I really want to make sure, that I can do that if they are submitted. I was assuming I just list it under the category Primary autograph. Also after I posted I went to do some more research. I originally thought they charged a shipping fee of $17.50 and a insurance fee. I can not find the insurance fee listed so I am curious if the insurance fee is include in the price of shipping, which would make more sense. Thanks again for all the help.
