    The address on the back of any Upper Deck product is the one I used when I sent back some cards.

    It is:

    The Upper Deck Company, LLC
    5909 Sea Otter Place
    Carlsbad, CA 92008

    I always sent a brief description of where and when I bought the pack and even had my local card dealer send a note with mine so they knew it was legit.
