Question about Upgrade/Reholder

Suppose there is a coin in an Old Green Holder that you think has a good chance of upgrading and would be worth the cost to try.

The PCGS website only says:
Regrade: Use the "Regrade" service for coins previously graded by PCGS that you feel might be worthy of a higher grade. Submit the coins in the holder. We guarantee that the coins will not be downgraded.

Does PCGS have to remove the coin from the holder in order to grade it? Does it matter if there are scratches on the OGH? Basically, if I submit the coin and it comes back with the same grade, will it come back in the OGH or a new slab?

No matter what the OGH will be gone when you get it back.

Yep, what angryturtle said. Unfortunately I sent in a few OGH (even some yellows) and rattlers for re-grade when I was young and stupid (ie. not that long ago) before I realized the attractiveness and desire for these older holders. Many came back with the same grade, all in a shiny new holder.

It comes back in a new holder. I believe they state on their site that they crack the coins out for the submission and then grade and reholder.

As has been stated, they crack the coin out and grade it raw. However, they guarantee that it will not downgrade. Regardless, it is returned in a new holder with a new certification number.
