OT: Need help with writing a tax receipt

Hi all. I promised a few folks on the board that I would write them a tax receipt for their donations of cards to my school. I am looking around online for a template or something, but I am not finding anything "definitive" that makes me comfortable that I am sending out the right thing. Is anyone here a tax accountant (is that a term?) or something who could tell me what I need to put on a receipt for these good people? Thanks!

If I were an auditor, I would expect something that lists the name of the school, contact information, tax ID number, date of contribution, description of contribution, and the name and signature of a school official (principal or vice-principal - anyone with the authority to approve and accept donations in a high-level position within the organization).

You don't want to assess the value of the gift - it's the responsibility of the donor to determine value.
