My 2011-W $50.00 Buffalo got 96ed

I sent in the COA, and Bill of Sale, and it was in it's mint plastic.
I didn't send it in the Mint's brown box. Is this why?

96 is the code for large and prominent scratch(s).
I decided to change calling the bathroom the John and renamed it the Jim. I feel so much better saying I went to the Jim this morning.

    The name is LEE!

    96 says refund,no service.
    Scratch is a 95.

No, that shouldn't have mattered. Weird.

I know with the 25th ASE sets, PCGS split it into 2 orders, one was the coins, and the other was the mint items such as COA or box. The mint items will come back as 96 because they aren't being graded. Are you sure the 96 was for the coin and not some mint items you sent in with the coin?
