Help Explain 1999 State Quarter Error?

Anyone know what caused this state quarter error?

Some background on the coin, my brother works at a credit union where a member brought it in and it got stuck in the coin counter obviously due to the thickness.

These are his photos and I haven't actually handled the coin. I can get more info from him if needed. Thanks for the help!

    It's not an error coin.


    It's Post Mint Damage -

    It's been 'tumbled', or otherwise
    tapped, hit, damaged, etc.......

    A fairly common effect -

    I've seen those but always wondered how they get the hole in the middle. image
    There's a guy who sells rings made out of Canada half dollars on eBay and you can still see the design elements of the coin.
