Pack submission form, please help

So a bunch of us are taking advantage of the pack grading special. I know how to send in a 100 plus cards for grading and using the additional forms, but what about for pack grading.

Has there been another form created when you are subbing more than 20 packs?

I've sent in more than 20 packs before, and used an excel spreadsheet for the others.

If it's easier, just fill out 5 different sheets (1 of 5, 2 of 5, etc) -- might even be easier to keep track of the various sellers/packs
