Did this OGH turned in the slab from light green to light blue?

Yes. And it's not that unusual.

Either that or your the white balance was off on the photo.

My money is on this, for the most part... bet the color faded a bit, and the white balance threw everything else off.

Yes. The early OGH holders did not have color stable ink used in the insert. Some of these turned blue over time and others have turned bright yellow.
