What's up with this 2011-W AGE Unc Submission???

I've got questions.

1. I thought that the $50 Unc. was the only coin available with the 'W' Mintmark.
2. Did this customer buy these coins in 4 pc. sets?
3. Did the Mint do a poor job on minting the $10 and $5 AGE's???
4. Would you be happy with this submission??? image

    Maybe Proofs that were keyed in wrong by a PCGS clerk??? Or maybe a super secret submission of a Mint employee who made them and snuck them out of the Mint last weekimage

    Gotta be proof sets that were submitted for first strike in a sealed box, otherwise customer would have seen the dings. Only one 70 out of 20 sealed mint coins, bummer - bad mint.
