Help with 50+ card submission

Newbie question: I will be sending in my first 50+ card sub, and have a question I was hoping someone could answer. The instructions specify one sub form per service type, but the form only has 20 rows. Do I use three separate submission forms (with 3 different #s) to fit all of the cards, or is there a better way to do it. It seems that all 50 cards should be under one submission # in order to get the 50 card special rate.

This piece of the submission is not something to be scared about screwing up. Use your own spreadsheet with 50 rows, print it out, and staple it to the form. Works every time.

I second this. I have not actually listed my cards on the official submission form since my very first CC submission many moons ago. Go the spreadsheet route for the itemized list, then write 'see attached spreadsheet' across the table on the sub form. Just fill out the money stuff on the official form.
