Looks like a retained cud - happens when a die crack extends from the rim back to the rim and is just about ready to break out and form a cud, but barely hangs on and keeps striking coins.

I've seen this effect before, also on the rim - I believe it's a small sliver of copper that either remained from the upsetting process, (on the planchet) or a detached sliver of copper that found its way into the coining chamber.
It's not a die error of any type, imo -

I think Fred's opinion is needed too.
