Looks like PMD to me. Notice how the T & Y are mushed down.

image My guess would be put into a vise at one time. Is there crushing on the other side also?
  • With the T and Y being "mushed down", that could happen with it being a struck through error, as the coin would not be able to be properly stuck up. If it were put into a vice, there would be way more damage than that.

As for exactly what happened, I don't know.

If it were struck through "cloth," the "T" would show significantly less detail as the "cloth" pattern extends near the top of the "T." Same for the area around the "Y." Whatever it is, it is not a strike through. Likely PMD of some fashion. From the 2-dimensional images I would have just guessed weird toning.
